The Community Matrons Team support housebound patients with multiple health and social care needs to prevent unnecessary hospital admission and provide better health outcomes.
This role is Mon-Fri 9am – 5pm across Westminster.
- Registered nurse or nurse associate
- Current NMC registration
- Experience of community health
- Experience of monitoring patients with complex conditions and identify deterioration
- Ability to undertake vital signs observations including urinalysis, weight, MUST scores
- Venepuncture skills
- Strong communication, administration and documentation skills
- Time management skills
- Knowledge and experience of using S1
- Ability to work independently and remotely from colleagues
- Ability to work across various locations in Westminster (this often requires walking with equipment in trolley bag)
