Job Summary

Welcome to FCMS!

We are looking for a friendly and passionate Wound Care Nurse to join our busy Doncaster Team.

FCMS pride themselves on being part of the Doncaster Wound Care Alliance, we currently run clinics six days a week from the Same Day Health Centre and hold twice weekly complex lives walk in wound care clinics. This has a huge impact on patients who are unable to attend routine appointments due to their complex lifestyle. The wound care team is currently made up of 2 specialist wound care nurses, a healthcare assistant and will soon include a Nurse Associate.

The City of Doncaster now exceeds 300,000 people who can all benefit from the service we offer. Patients are referred into our service through their own GPs, the emergency department, triage, or pharmacy. We care for wounds from superficial burns to complex Tier 3 leg ulcers.

Pay: £19.10 - £23.00 depending on experience, with additional enhancements for nonmedical prescribers

Main duties of the job

Main Duties Of The Job:

Provide expert wound care to all patients in line with the Wound Care Alliance training.

Give clear and concise communication to service users.

Liaise with secondary care providers and primary care to ensure continuity of care and referrals as appropriate.

Practice safely and effectively with the patients best interest at the core, work within own limitations and seek expert help and advice when needed

Demonstrate a sound understanding to clinical assessment and advice relating to the treatment and management plan in line with local formularies.

Ensure all mandatory training is up to date and have willingness to develop with further CPD.

Support the senior management team and clinical manager with service improvement.

Mentoring colleagues.

Act in accordance with the NMC.

Provide professional and clinical advice within the multi-disciplinary team.

Job responsibilities

Role

Following the completion of the Wound Care Alliance training the post holder will manage the following presentations:

Skin tears

Post-operative wounds

Burns

Self-harm

Leg ulcers

Cavity wounds

Lower limb assessments for compression

The Post Holder Will Also:

Engage with telephone triage

Provide face to face assessments

Provide a clear follow up plan and safety netting after each appointment

Demonstrate flexible working in line with the need of the service, including home visits

Escalate and discuss any clinical concerns with senior clinicians and the clinical manager

Keep up to date with mandatory training

Maintain competence in adult and childrens safeguarding

Provide peer support

Engage and evidence CPD

Mentor trainee nurse associates and healthcare assistants

Attend meetings when required

Assist with appraisals

Promote the service and implement changes in line with Doncaster CCG

Promote patient education on nutrition and scar management

Work with the team to book patients into clinics as appropriate

Knowledge, Skills, And Experience Required:

Experience of working within wound care is desirable. The post holder will demonstrate good professional work ethic and a willingness to learn. They will demonstrate good clear communication skills and the ability to work autonomously and within a multi-disciplinary team. Treatment plans will be in line with local formulary and with the patient at the centre. The post holder will engage in continuous CPD throughout their employment and have a willingness to share experience and teaching with the wider team. The post-holder will be expected to undertake all the duties and responsibilities associated with a wound care nurse within urgent and emergency care. The post holder will ensure all aspects of safeguarding are considered in each patient contact and act appropriately with local and organizational guidelines.

Duties and Responsibilities

The Post Holder Will:

Role model standards of care and behaviour through clinical practice.

Act as a role model for standards of behaviour and professionalism, through commitment to the integration of policies and procedures within the role and workplace

Analyse relevant local and national policy, within their area of expertise and advice on the impact for service, leading the implementation of changes and evaluation of processes

Maintain professional registration and practice through CPD and revalidate as required

Maintain a professional portfolio to demonstrate competency

Maintain their personal standards of conduct and behaviour consistent within FCMS and the NMC.

Allocate, coordinate, monitor and assess own workload within an accepted time frame

Ensure appropriate use of resources in order to meet service needs

Recognise and act as an advocate for patients, carers, service and the organisation

Promote workforce integrity (empower professional accountability within the clinical team, ensure accreditation process followed, complaints management, competency assessment, ensuring mandatory training requirements fulfilled, robust clinical induction).

Maintain a sound knowledge of NHS policy and NICE guidance in relation to scheduled and unscheduled care.

Take part in a driving assessment and always demonstrate safe risk averse driving in line with FCMS policies.

Maintain up to date clinical knowledge on the assessment and treatment of wounds.

Provide wound care services to the people of Doncaster.
